If you just prefer to get the refund, please simply answer the followings questions, so we can process the refund.
1. Have you already uninstalled ez1095? (if not, please uninstall it first to request refund.)
2. Do you agree to purchase license key again if you decide to use ez1095 again in the future? (It is illegal to use our software after we issue the refund.)
3. Have you printed or efile any forms and issued them to your employees, clients or IRS? (All these forms are with our company vendor ID.)
4. Have you shredded all the forms you printed?
